Osteopathic Physician (DO) Application

For DOs in practice, in the military, in the public health service, in internship or residency/postdoctoral training or retired. Download application (PDF, 56K)

Associate Member Application

For those who are not Osteopathic Physicians (D.O.s) but have a professional relationship with an AOA D.O. member and/or with affiliates of the AOA as specified in the bylaws found at the end of the application. Download application (PDF, 152K)

Allied Member Application

For those licensed healthcare providers who are currently employed with an active member of the AOA, contribute to the practice of that member, are not eligible for any other category of membership and who support the goals and objectives of the AOA or For physicians holding an MD degree and licensed to practice in the US who support the AOA mission and subscribe to its code of ethics. International Physician Application

International Physician Membership in the American Osteopathic Association is limited to those MDs who are graduates of schools of medicine located outside of the US on an official list of schools recognized by the AOA, holding a license for unlimited scope of medical practice including the authority to prescribe without limitation in their country of practice, and reside and practice outside the US and support the goals and objectives of the AOA and agree to subscribe to its code of ethics.

Application for membership and payment of any fees should not be construed as acceptance into International Physician Membership. The Committee on Membership reviews applications for membership and provides appropriate recommendations to the Board of Trustees for action.
